Background and Aim: In Germany, the discharge medication is usually reported to the general practitioner (GP) by an inital short report (SR) /notification (handed over to the patient) and later by a more detailed discharge letter (DL) of the hospital. Material and Method: We asked N=536 GPs (from Frankfurt/Main and Luebeck) after the typical report format of their patients discharge medication by the local hospitals. The questionnaire asked for 26 items covering (1) the designation of the medication (brand name, generic name) in SR and DL, (2) further specifications e.g. possibilities of generic substitution or supervision of sensible medications, (3) reasons why GPs do not follow the hospitals recommendations and (4) possibilities for an improvement in the medication-related communication between GP and hospitals. Results: 39% GPs responded sufficiently to the questionnaire. The majority of the GPs (82%) quoted that in the SR only brand names are given (often or ever) and neither the generic name or any further information on generic substitution is available (seldom or never). 65% of the responders quoted that even in the DL only brand names are given. Only 41% of the responders quoted that further treatment relevant specifications are given (often or ever). 95% responded that new medications or change of custom medication is seldom or never explained in the DL and GP were not explicitly informed about relevant medication changes. 58% of the responders quoted economic reasons for re-adjustment of the discharge medication e.g. by generic substitution. The majority of responders (83%) are favouring (useful or very useful) a pre-discharge information (e.g. via fax) about the medication and 54% a hot-line to some relevant person in the hospital when treatment problems emerge. 67% of the responders quoted in favour of regular meetings between GPs and hospital doctors regarding actual pharmacotherapy. Conclusion: In conclusion, our survey pointed to marked deficiencies in reporting the discharge medication to GPs. Conflict of interest: Non

Understanding the Role of Data for Innovating Business Models: A System Dynamics Perspective

    Get PDF
Data have become a key ingredient for ICT-enabled business models. Nevertheless, there is great uncertainty among scholars and practitioners alike about how to leverage data as an essential innovation resource. This raises the question of how to design data roles to innovate business models. To answer this question and to facilitate a deeper, cause-effect-relation understanding of the interdependencies between data roles and business models, system dynamics was chosen as the approach of analysis. Within a multiple case study of five business model cases with multiple embedded units per case, the study shows that there are four recurring basic data role patterns e.g. ‘incremental improvement’ or \u27initial data boost’ and two data role characteristic patterns, describing how data roles unfold within business models e.g. ‘change in self-reinforcement’ pattern. Overall, the patterns help to visualize and articulate data usage in business models and therefore contribute to the ongoing endeavor of innovating business models

Towards Open Innovation in Embedded Systems

    Get PDF
By opening up a technology, firms can incorporate external actors to improve their products or to develop new applications on top of it. This phenomenon is discussed in the literature as open innovation (Chesbrough, 2003). An embedded system is an example of a modular and traditionally closed technology where parts of the system can be opened in order to stimulate innovation. In this paper, we want to explore how firms can pursue the opening of their embedded systems and their organization in order to unleash the potential of open innovation. By conducting twelve explorative interviews with experts in the field of embedded systems, we discovered three different forms of embedded systems openness and explored technical, internal and external organizational challenges associated with these three forms of openness

Making Sense of the Sustainable Smart PSS Value Proposition

    Get PDF
While academia attributes superior value potential to sustainable smart PSS (SSPSS), in practice, they are not widely implemented. To address this gap, we analyze how the notion of SSPSS value is constructed through sensemaking. Adopting a case study approach, we explore differences in organizational sensemaking. Moreover, we analyze how the three functional roles “digital innovation and technology”, “sustainability”, and “market” involved in innovating SSPSS make sense of the value proposition. We conclude that value is subjective and the value proposition of SSPSS is multi-faceted. Each facet is constructed through the interaction of organizational, functional roles’, and individual sensemaking. At the organizational level, commitment, identity, and expectations influence the creation of shared meaning. At the functional role level, actors differ in their sensemaking based on the cognitive frames applied. At the individual level, subjective beliefs impact sensemaking. Hence, sensemaking is a multi-level process that raises the question of alignment

Remote Sensing-Based Methodology for the Quick Update of the Assessment of the Population Exposed to Natural Hazards

The assessment of the number of people exposed to natural hazards, especially in countries with strong urban growth, is difficult to be updated at the same rate as land use develops. This paper presents a remote sensing-based procedure for quickly updating the assessment of the population exposed to natural hazards. A relationship between satellite nightlights intensity and urbanization density from global available cartography is first assessed when all data are available. This is used to extrapolate urbanization data at different time steps, updating exposure each time new nightlights intensity maps are available. To test the reliability of the proposed methodology, the number of people exposed to riverine flood in Italy is assessed, deriving a probabilistic relationship between DMSP nightlights intensity and urbanization density from the GUF database for the year 2011. People exposed to riverine flood are assessed crossing the population distributed on the derived urbanization density with flood hazard zones provided by ISPRA. The validation against reliable exposures derived from ISTAT data shows good agreement. The possibility to update exposure maps with a higher refresh rate makes this approach particularly suitable for applications in developing countries, where urbanization and population densities may change at a sub-yearly time scale

We are currently witnessing a multiplication of “educations for” which explicitly aim to change behaviors. But by what right should these behaviors change? And in the name of what to legitimize new ones? This is the question of justification of “educations for” and their normative dimension which arises here. Borrowing from both the Anglo-Saxon analytical approach and the epistemology of Canguilhem, this article aims to justify a didactic strategy specific to “educations for”. To do this, we will rely on an analysis of the concept of “norm”, which reveals the role of “anti-values” in determining the meaning of values. Heritage education, wedged between an absolutist justification and a relativistic justification, will illustrate our point

Unexpected role for the human Cx37 C1019T polymorphism in tumour cell proliferation

Connexins are a large family of proteins that form gap junction channels allowing exchange of ions and small metabolites between neighboring cells. They have been implicated in pathological processes such as tumourigenesis in which they may act as tumour suppressors. A polymorphism in the human connexin37 (Cx37) gene (C1019T), resulting in a non-conservative amino acid change in the regulatory C-terminus (CT) of the Cx37 protein (P319S) has been suggested to be implicated in predisposition to angiosarcomas. In this study, we have used communication-deficient HeLa and SK-HEP-1 cells transfected with Cx37-319S, Cx37-319P or empty vector. We showed that the expression of Cx37-319P limited proliferation of HeLa and SK-HEP-1 cells, whereas Cx37-319S expression was without effect. Using an in vitro kinase assay, we demonstrated phosphorylation of Cx37 CT by glycogen synthase kinase-3 (GSK-3), a kinase known to be implicated in cell proliferation and cancer. GSK-3-induced phosphorylation was associated with reduced gap junctional intercellular communication (GJIC) as measured by microinjection of the tracer neurobiotin. Inhibition of GSK-3 by LiCl or SB415286 reduced phosphorylation of Cx37-319P and increased GJIC. This latter effect on GJIC involved the beta and not the alpha isoform of GSK-3. In contrast, GSK-3 inhibitors were without effect on HeLa cells expressing Cx37-319S. In conclusion, our data indicate functional effects of the Cx37 C1019T polymorphism on GJIC that might contribute to tumour cell growt

On the way to the "best childrensÂŽs university": The supply and demand relationship under the principle of partcipation. Opportunities and challenges raised by the childrenÂŽs university

I know the children’s university is the best university in the world,” says Elisabeth, who is 9 years old and an attendee at the children’s university in Vienna. This charming compliment paid to the children’s university organizers raises an opportunity for further critical inquiries. The statement puts “real” universities on a par with children’s universities and therefore contains the implicit question of the status of children’s universities in the educational landscape: as a hybrid of formal and non-formal education. The specialist conference “On the way to the ‘best Children’s University’: The Supply and Demand Relationship under the Principle of Participation. Opportunities and Challenges”, took place from 8 to 10 May at the University of Cologne, addressed this intermediate position in all its facets. Further topics were possibilities for getting children involved as equal partners, and an international comparison of children’s university models. The conference was funded by the Ford Foundation and supported by KölnAlumni – Freunde und Förderer der UniversitĂ€t e.V. It was organized in partnership with the international children’s university network EUCUNET along with the multinational EU project “SiSCatalyst. Children as change agents for science and society”

The Lyman alpha reference sample. VII. Spatially resolved Hα\alpha kinematics

We present integral field spectroscopic observations with the Potsdam Multi Aperture Spectrophotometer of all 14 galaxies in the z∌0.1z\sim 0.1 Lyman Alpha Reference Sample (LARS). We produce 2D line of sight velocity maps and velocity dispersion maps from the Balmer α\alpha (Hα\alpha) emission in our data cubes. These maps trace the spectral and spatial properties of the LARS galaxies' intrinsic Lyα\alpha radiation field. We show our kinematic maps spatially registered onto the Hubble Space Telescope Hα\alpha and Lyman α\alpha (Lyα\alpha) images. Only for individual galaxies a causal connection between spatially resolved Hα\alpha kinematics and Lyα\alpha photometry can be conjectured. However, no general trend can be established for the whole sample. Furthermore, we compute non-parametric global kinematical statistics -- intrinsic velocity dispersion σ0\sigma_0, shearing velocity vshearv_\mathrm{shear}, and the vshear/σ0v_\mathrm{shear}/\sigma_0 ratio -- from our kinematic maps. In general LARS galaxies are characterised by high intrinsic velocity dispersions (54\,km\,s−1^{-1} median) and low shearing velocities (65\,km\,s−1^{-1} median). vshear/σ0v_\mathrm{shear}/\sigma_0 values range from 0.5 to 3.2 with an average of 1.5. Noteworthy, five galaxies of the sample are dispersion dominated systems with vshear/σ0<1v_\mathrm{shear}/\sigma_0 <1 and are thus kinematically similar to turbulent star forming galaxies seen at high redshift. When linking our kinematical statistics to the global LARS Lyα\alpha properties, we find that dispersion dominated systems show higher Lyα\alpha equivalent widths and higher Lyα\alpha escape fractions than systems with vshear/σ0>1v_\mathrm{shear}/\sigma_0 > 1. Our result indicates that turbulence in actively star-forming systems is causally connected to interstellar medium conditions that favour an escape of Lyα\alpha radiation.Comment: 26 pages, 15 figures, accepted for publication in A&
